You may either fill out the application on the web, print it out and mail it, or you can print it out, fill it out by hand and mail it in. At this point I am unable to have this file submitted digitally. Sorry for the inconvience.

Tuition Payment Plan for class series:
The cost for the 12 week classes is $325—$275 for the classes and $50 for materials which includes research, handouts and a candle lit dinner week two. I offer three payment plans for your convenience. The cost for materials must be paid in full with the first payment. Please select which plan works best for your family.

• One-time payment of $325 (If paid before the start of class, there is a 5% discount, total of $308.75)
• Two-time payment: $187.50 ($137.50 + $50 materials) due week 1, $137.50 due week 6
• Weekly payments*: $75 ($25 + $50 materials) due week 1, $25 each remaining class

*The cost for the weekly payment plan is slightly more than the other two plans.

Failure to attend classes, due to vacation or sickness, does not relieve couple of their obligations to pay for the 12 classes in full. Refunds for unattended classes are offered only in the case that the pregnancy ended early and the couple was no longer in need of the classes or the mother was placed on bed rest and was unable to attend classes.
